6. Overseas Medical Policy.
7. Return Air ticket.
8. Insurance as per Travel Dates.
9. Passenger’s personal IT last 3 years.
10. Salary slip last 3 months.
11. Passenger’s personal bank statement last 6 months.
12. Appointment letter copy.
13. Company Registration Copy.
• Tourist Visa
1. Original 6 months Valid passport
2. One visa application form online filled With Appointment Date.

5. Hotel confirmation from Poland.
6. Overseas Medical Policy

10. Passenger’s personal bank statement last 6 months, If Employee than required Leave letter from company.
11. If Director or Proprietor Then Memorandum of the Company or Gumasta License or Any Proprietorship Proof.
